Home Assessments:
Why pay for two separate assessments for mobility needs and equipment? – our physiotherapists have the scope of practice and extensive experience to do both!
Yes, we can assess your home, your function and clarify your needs, and also make recommendations and prescribe aides, equipment and home modifications.
In- home physiotherapy service, specifically targeting issues that can affect you as you age:
Falls risk reduction – Balance and reaction time
Maintaining and regaining ability to get up from the floor
Strength for activities such as climbing steps, ramps or getting up from low chairs
Post-fall rehabilitation, reablement and mobility training
Resistance training to combat bone and muscle loss
Regaining fitness and mobility after serious illness
Falls risk and home safety assessment
Dementia Friendly Environmental Assessments:
Bringing the National Aged Care Design Principles & Guidelines into the home to best support those living with dementia
Goods, Equipment & Assistive Technology Assessment:
Have a CHSP code for Goods, Equipment & Assistive Technology but can’t get a CHSP OT to do the assessment. We can come to your home, assess and lodge the request for the equipment for a rate comparable to a CHSP OT visit (Private Health rebates apply)
If you have a maxed out Home Care Package, you can also access additional support with Goods, Equipment & Assistive Technology – ask us how!
Manual Handling Training:
With over 20 years providing manual handling advice in hospitals, aged care, healthcare and workplace settings, our physiotherapists can ensure you have the right equipment, documentation and practices in place to keep staff, carers and clients safe.
Jennifer Holmes is a physiotherapist with over 25 years experience, and is passionate about people living their best lives as they get older. She has a special interest in working with clients to clarify and then work towards their mobility and health goals.
She also has extensive experience in home assessment, task modification and equipment prescription and can make recommendations to keep you safe and happy in your home and community as you age.
Up & Go Physio is an approved Provider with a number of Home Care Packages and can work with your home care provider to be paid under your package.
We also have a Medicare provider number to claim services on your private health or you can engage us privately.
